How to fill out your new patient paperwork
BEFORE you come to our office!
- If you do not already have AdobeReader® installed on your computer Click Here to download.
- Forms marked [Required] have to be filled out by every patient; forms marked [Workers Comp] or [Personal Injury] are only for patients who will be filing through those methods.
- Download the necessary form(s), print it out and fill in the required information.
- Fax us your completed form(s) 570-287-5575, or bring it with you to your appointment.
- Call the office 570-287-7070 if you have any questions!
Patient Demographics Form [Required]
This will provide us with your current contact information, general health history, current state of your injury or condition, and any additional information about supplements, or other medications you may currently be taking. Basically, just tell us what we can help you with - and we will get you out of pain, now!
Family Health History Form [Required]
This will give us a general idea about conditions or diseases that may be affecting your immediate family members.
Insurance Intake Form [Required]
This will provide us with detailed information about your insurance company and current plan. Also provides the insurance department with the insured persons' information so that we can file your claims accurately. This is very important so your insurance company doesn't have a problem processing your claims, which means faster service for you!
